If your student requires a medication for an allergy including food or environmental, asthma, seizures,etc please click on the specific action plan for your student on the left hand side and fill out the action plan. These action plans are to be signed by parent/guardian, student's provider and returned to the health office along with the medication and medication order if your student requires a medication.
All action plans must be renewed annually at the beginning of the school year.
It is recommended that all students with a health condition of asthma, seizures or anaphylaxis allergy have a action plan on file annually and emergency medications kept in health office.
If a student requires the health office to remain in contact or to request any orders/medication with student's primary provider, a Release of Medical Information is to be filled out and returned to health office (See link below)
